Managing your plan

Eligibility
Our services are available to full time employees and retirees, an employee's legal spouse and/or an employee's children and an enrolment form must be completed for each individual (employee & dependant).

Changes and amendments to your plan
We require immediate notification for any changes in employee status, terminations/leavers, changes in life benefits, newborn children to be covered under the plan, extension of dependant child coverage, changes in beneficiaries and/or death of any individuals covered.

Accessing your healthcare
Pre-certification is required for all hospital confinements, all major out-patient procedures and/or overseas treatment.

Making a claim

For all claims, you should submit the following documentation within six months of the incurred date:

  • Original claim form certified by attending physician (incl. diagnosis and relevant CPT codes).
  • All related bills and receipts.

How to make a medical, dental or vision claim

  1. Complete a separate claim form for each insured member making a claim.
  2. Complete a Health Insurance Claim Form for medical expenses, an Attending Dentist's Statement for dental claims and/or a Vision Claim form for vision claims.
  3. Ensure all claims being submitted include a diagnosis and relevant CPT codes for each procedure performed by the attending physician, the insured and claimant's signatures and/or all related bills and receipts.
  4. Submit claims to Generali Worldwide within 6 months of the date incurred.
